Group of happy medical teamAt St. Joseph’s Care Hospice Services, Inc., we have a team of hospice professionals, including on-call nurses who remain available 24 hours a day to respond to needs. The goal of this specialized team is to help you to live each day to the fullest. The team includes:

  • Hospice Medical Director
    A dedicated hospice physician who works closely with your personal physician to manage any distressing symptoms and improve quality of life.
  • Registered Nurses
    Work closely with the patient’s physician and other team members to monitor the patient’s status and effectively manage pain and discomfort.
  • Licensed Vocational Nurses
    Under the guidance of a Registered Nurse, the LVN provides services following the hospice plan of care.
  • Hospice Aides
    Provide assistance with personal care including bathing, dressing, toileting, and other activities of daily living. May also assist with some light housekeeping tasks while providing care.
  • Social Workers
    Provide emotional support to the patient and family. They can also assist families in locating appropriate community resources related to their specific needs. Offer a supportive ear to patients and their family members to discuss concerns surrounding death, end of life planning, resolving relationships and dealing with feelings of grief, anger, frustration and uncertainty.
  • Spiritual Care
    Assist patients and their families with understanding and coping with spiritual concerns. Instrumental in helping to make contact with the client’s personal house of worship, if requested. These individuals support a variety of different belief systems, and their hospice role, are not affiliated with any one particular religion.
  • Bereavement Counselors
    Keep in touch with the patient’s loved ones during the first year following the death of the patient. Offers letters, support groups and counseling when needed. Invite family members to an annual memorial service to remember their loved one.
  • Volunteers
    Visit the patients in their homes to provide support and companionship. These individuals might serve as a friendly visitor, sit alongside the patient, conducts pinning ceremonies for our country’s Veterans. Volunteers are the heart of the hospice. In fact, hospices in the United States were founded by volunteers.
